We use submissions to respond to you, understand community needs, route volunteer and partnership inquiries, and guide Foundation planning and design.
What not to send
Please do not submit medical records, Social Security numbers, financial account information, passwords, or other highly sensitive identifiers through these forms.
Sharing
We do not intend to sell submitted information. Information may be shared internally with authorized Foundation personnel or service providers as needed to operate the forms and respond to submissions.
